1. ASSUMPTION OF RISK

I understand that using golf simulators and participating in activities at The King’s Tee involves risks such as physical injury, equipment malfunction, and potential accidents. I voluntarily assume all risks associated with these activities.

2. RELEASE OF LIABILITY

I release and discharge The King’s Tee, its owners, staff, and affiliates from all liability for any injury, damage, or loss, including those caused by negligence, that may occur as a result of my use of the facility or participation in activities.

3. EQUIPMENT USE

I agree to use all equipment responsibly and only for its intended purpose. I will report any damage or unsafe conditions immediately.

4. RULES AND CONDUCT

I agree to follow all safety rules and directions from staff. I understand that failure to do so may result in removal from the facility.

5. MEDICAL TREATMENT

In case of an emergency, I authorize The King’s Tee to seek medical attention on my behalf and accept responsibility for any costs.

6. MINORS

(If signing for a minor) I am the parent/legal guardian and accept all responsibility for the child’s participation and compliance with this agreement.

7. MEDIA RELEASE

☐ I grant permission for photos/videos of me (or my child) to be used in promotional materials.
☐ I do not grant permission.

8. BINDING AGREEMENT

This agreement is binding upon me, my heirs, and legal representatives. I acknowledge that I have read, understood, and voluntarily agree to the terms.
